This printable 12-Week Health & Fitness Journal Kit consists of:
- Instant digital download: One PDF (A4) with a full 12-week suggested layout (65 pages), plus individual page files if you wish to create your own journal layout instead.
This downloadable kit includes 14 pages plus a cover page that you can print as many times as you want. They are a great, budget-friendly way of keeping track of your health, fitness and wellbeing goals. These pages include:
★ Goal Setting Tips – An information sheet on how to create goals you’re more likely to achieve and how to review things on an ongoing basis to keep you moving towards them.
★ 12-Week Overview – To reflect on your key weekly goals for the 12 weeks ahead.
★ Getting Started – Identify your big reason why you’re looking to make lifestyle changes and map out how you’re going to do it.
★ Measurements Log – Tape measurements and clothing sizes to track your progress. Please note: ‘Weight’ has been purposely left off this log sheet as it is not an absolute indicator of health and often doesn’t help to monitor!
★ Fitness Progress – For an alternative/addition to tracking physical measurements.
★ Inspiration & Motivation – Motivational quotes sheet to help get you through tough times and keep you looking ahead toward your goals.
★ Habit Tracker – A space to check off the days in a month when you have specific habits you’re working towards that require daily tracking.
★ Goal Setting – SMART goal worksheet to help you figure out what you want to achieve and how to get there.
★ Goal Review – To review your progress (from the Goal Setting worksheet), highlight your achievements and identify any changes needed for the next period.
★ Exercise Planner – Assess your timetable and plan how you’re going to get active on a daily basis.
★ Food Planner – Plan your weekly meals and snacks, and develop a list of quick and healthy ‘go to’ options.
★ Weekly Diary – A weekly overview to take note of your food and water intake, exercise sessions, energy levels, mood, and goal progress as you go through your week [2 page spread]. Please note: As I don’t advocate counting calories/blocks/points etc. for food intake, this is more of an area to log what you’ve eaten to gain more awareness of your eating habits.
★ Final Reflections – To go back over your 12-week journal and assess the progress you have made and what changes you want to continue in the future.
How to use the planner
Get an A4 folder such as a ring binder or clearfile to hold all your printouts and use whichever individual sheets you feel fit with your goals. Alternatively, you can print the fully assembled PDF version (65 pages) which has several pages of habit tracking at the beginning (for easier access), along with goal setting and reviews spaced every four weeks.
If you are doing a health or fitness challenge that lasts a set number of weeks you might like to get the pages printed at your local copy store and have them bound.
